Overview
What is a global surgery period?
The global surgery period (or global period) is the span of time around a procedure during which Medicare considers routine, related care to be part of the payment for the surgery itself. During this window, typical follow-up visits and normal post-operative care are not billed separately, as they are already included in the surgical fee.
Every procedure code carries a global-period indicator that tells you how long that window lasts. The three that produce a date range are 000, 010, and 090 days. This Global Surgery Calculator reads that indicator from the fee schedule and turns your surgery date into the exact global-period start and end dates.
How the calculator counts the days
- 0-day (000): the day of the procedure only.
- 10-day (010): the day of the procedure plus the next 10 days.
- 90-day (090): one pre-operative day, the day of surgery, and the following 90 days.

What is a 90-day global period?
A 90-day global period applies to major surgery. It includes one pre-operative day (the day before surgery), the day of surgery itself, and 90 post-operative days, for a total span of 92 days.
Does the day of surgery count in the global period?
Yes. For 10-day and 90-day global periods, the day of the procedure is included. The 90-day period additionally includes the single day before surgery as the pre-operative day. The 0-day (000) global period is the day of the procedure only.
What do XXX, YYY, ZZZ, and MMM mean?
XXX means the global surgery concept does not apply. YYY means the global period is set by your Medicare Administrative Contractor. ZZZ is an add-on code whose global period follows the primary procedure. MMM identifies maternity codes, to which the usual global concept does not apply.
